Inspector Notes

An unannounced, on-site Subsidy Health and Safety Inspection (SHSI) was initiated on 08/29/24 and completed on 08/29/24. The on-site inspection began at 10:45am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were four children present and two caregivers; the point total was 10. The inspector reviewed five children?s records and two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

A renewal inspection was also conducted on this date. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in the licensing standards are reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on a review of documentation and interviews, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: 1. The record of daily attendance for the week of August 26, 2024, was reviewed on 08/29/24. The record of daily attendance did not document the arrival and departure of each child in care that week. The record of daily attendance for 08/29/24 did not document the arrival of each child in care that day. The inspector observed four children in care.

2. During interviews, Caregiver #1 acknowledged the arrival and departure of each child in care has not been documented.
Plan of Correction: Moving forward, the provider will document the arrival and departure of children as it occurs.
