During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 03/11/2026, as a part of the licensure period. The on-site inspection began at 11:15am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 12 children present and 3 caregivers; the point total was 30. The inspector reviewed 5 children?s records and 3 caregiver and 2 household members records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years.

The provider did not obtain a TB screening for household member #1 and #2 every two years.
Plan of Correction: Both household members have appointments scheduled to have their TB screenings completed.
Standard 8VAC20-800-230-B
Written documentation of annual training shall include the name of the training session, date and total hours of the session and name of the organization that sponsored the training and the trainer.

Staff #2 and #3's documentation did not include the dates and name of all trainings completed over the past year.
Plan of Correction: The state model form for training documentation will be used and completed directly after each training staff complete.
Standard 8VAC20-800-780-B
The provider shall review with the parent the emergency contact information annually.

The last review of emergency contacts for Child #1 was overdue by 2 months.
Plan of Correction: This information will be reviewed with the parents.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care.

An emergency evacuation drill was not practiced during the month of February 2026.
Plan of Correction: Monthly fire drills will be completed.
