Inspector Notes

An unannounced, on-site renewal inspection was initiated on 10/22/25 and completed on 10/22/25. The on-site inspection began at 9:20am and ended at 11:20am. The inspector reviewed compliance in the areas listed above. There were 44 children present and 9 staff. The inspector reviewed 5 children?s records and 4 staff records on-site.
This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening prior to the first day of working with the children.

REPEAT

The PPD screening on file for staff #1 was completed 24 days after hire and working with the children.
Plan of Correction: A PPD screening will be obtained for future staff prior to hire/working with the children to ensure compliance.
