Inspector Notes

An in person renewal study was conducted today.
All rooms were in ratio during the inspection. Child and staff records, injury reports, first aid supplies, emergency evacuation drills, injury prevention plan, bottles labeled and dated, menu, and the playground were all observed today.
Violations discussed with the Director: Background searches, injury prevention plan update, injury reports. and TB documentation.
The report will be typed off site and emailed for signatures.

Based on a review of records, staff records do not contain documentation of a completed Criminal History Search for a residence outside of the state of Virginia in the past 5 years prior to employment.

Based on a review of records, staff records do not contain documentation of a negative Tuberculosis screening within the allowable time frame.
Evidence:
1 of 4 records reviewed did not contain documentation of a competed TB screening, negative results prior to employment, and not more than 30 days old. There are no results available for review. (Staff B)

Based on staff statement, there is no written injury prevention plan update available.
Evidence:
The injury reports have been reviewed and logged; however, there is no documentation to demonstrate the injury prevention plan was updated.
