During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/23/24, as a part of the licensure period. The on-site inspection began at 11:00 AM and ended at 12:20 PM. The inspector reviewed compliance in the areas listed above. CDC: There were 81children present and 19 staff. The inspector reviewed 5 children?s records and 3 staff records on-site
This inspection included: document review, tour of the facility, interviews and observations
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 1, 2024. A POC submitted after this date will not appear on the public website.

The center did not obtain, prior to employment, a sworn statement or affirmation disclosing whether the staff member has ever been convicted of or is the subject of pending charges for any offense within or outside the Commonwealth and whether he has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth.

Sworn Disclosure Statements (SDS) completed for Staff A, B and C were not obtained prior to employment.

The center is required to request background checks, prior to employment, from any state in which the individual has resided in the preceding five years.

1. The center did not request a search of the child abuse and neglect registry , and a criminal history search a residence out of the state of Virginia for Staff A and Staff B, prior to employment. The searches were 7 days after employment.

Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children. The documentation shall have been completed within the last 30 calendar days of the date of employment.

The screening for Staff C was completed more than one month prior to the date of employment.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Cleaning maerials were not stored locked in the Toddler 2 room, after diapering.

Repeat violation
For each infant, the center shall post a daily record that shall contain the amount of time in infant that cannot turn themselves over, spent on their stomach.

The tummy time log for one infant that requires time spent on their stomach, does not document the times the infant was placed on their stomach. (Infant 1)

Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

A staff member was observed using their hand to place a disposable diaper in the storage system for disposable diapers.
