During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 3/3/2026 and completed on 3/5/2026, as a part of the licensure period. The on-site inspection began at 1:50 PM and ended at 3:45 PM. The inspector reviewed compliance in the areas listed above. There were 78 children present and 20 staff. The inspector reviewed 7 children?s records and 3 staff records on-site. This inspection included: document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-245-J-3
Any child for whom emergency medications have been prescribed shall always be in the care of a staff member who has satisfactorily completed a training program for this purpose.

Child #1 who has been enrolled for 18 months and was present at the center, had an emergency medication on site. No staff members were certified in training to administer the medication.
Plan of Correction: Multiple staff will be trained by the end of the week.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain; parent's name, address and phone number, parent's work information.
Child #2's record did not contain a parent's name, address and phone number.
Child #7's record did not contain a parent's work information.
Plan of Correction: The parents have been contacted and asked to provide the missing information.
