Inspector Notes
An unannounced monitoring inspection was conducted from 9:30 - 10:30 am with staff. There were 31 children, ranging from four years to six years of age, present with four staff supervising. Children were observed practicing for graduation, doing kindergarten work and playing on the playground. Two children's records and three staff records were reviewed. One medication, along with it's authorization form and allergy care plan was reviewed and in compliance. The emergency drill log, attendance, written allergy list, fire and health inspections were reviewed. Based on review of two staff records, not all staff had a repeat tuberculosis (TB) test/screening at least every two years from the date of the previous test/screening. Evidence: the record for staff #1 had a TB test/screening dated 3/5/2020.
Plan of Correction: Will send the staff to get an updated TB test/screening.
Standard 8VAC20-780-40-M
Based on observation, and an interview with staff, the facility failed to have a current written list of all children's allergies, sensitivities, and dietary restrictions. Evidence: there is no written list of children's allergies and there is an enrolled child who has a diagnosed food allergy.
Plan of Correction: Staff stated they will create a written list of allergies, sensitivities, and dietary preferences.

Based on review of documentation, and an interview with staff, the facility failed to implement an annual lockdown drill to practice lockdown procedures. Evidence: there is no documented lockdown drill listed on the emergency drill log.
Plan of Correction: Will ensure all drills are documented on the emergency drill log.
