Inspector Notes
An unannounced monitoring inspection was conducted on 12/11/23 from 9:30 a.m. to 11:00 a.m. with the Director. There were 23 children in care, ranging in age from four to six-years-old, supervised by six staff. The children were observed in circle time. Four child records and six staff records were reviewed. The Center has six staff with current certification in CPR and First Aid, and six staff trained in Daily Health Observation Training. One medication and the authorization form were reviewed. Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery- operated radio were observed. The most recent Fire Inspection on file was dated 08/01/23 and the most recent Health Inspection on file was dated 08/08/23. Areas of non-compliance are identified in the Violation Notice. Violations
2Evidence:
1. On the playground, the wooden fence had four nails protruding out from the fence approximately one-half inch in length and nails were protruding from the bottom of the fence creating an impalement and scratch hazard.
2. The wooden fence had three warped boards, one of the boards had become separated at the bottom and was projecting toward the play area creating a trip hazard.

The drill log did not contain documentation of a drill conducted in November 2023.
