Violations

2
Standard 8VAC20-780-270-A
Based on observation, it was determined that not all areas of the center inside and outside were maintained in a safe condition.

Evidence:
1. On the playground, the wooden fence had four nails protruding out from the fence approximately one-half inch in length and nails were protruding from the bottom of the fence creating an impalement and scratch hazard.

2. The wooden fence had three warped boards, one of the boards had become separated at the bottom and was projecting toward the play area creating a trip hazard.
Plan of Correction: All nails will be hammered in and warped boards will be fixed.
Standard 8VAC20-780-550-D
Based on review of documentation and interview; the center did not implement a monthly practice evacuation drill.

Evidence: Repeat Violation

The drill log did not contain documentation of a drill conducted in November 2023.
Plan of Correction: Evacuation Drill will be done for December
