Inspector Notes

A Subsidy Health and Safety inspection was conducted on 11/15/2022 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on record review and interview, the Provider failed to ensure that caregivers who work directly with children shall annually attend the department's health and safety update course.

Evidence:
1. The record for Caregiver #1 did not contain a current annual 3-hour updated subsidy training course. The previous 3-hour update training course was completed in July 2021.
