During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated on 5/4/25 and completed on the same day, as a part of licensure period. The on-site inspection began at 10:10 and ended at 11:10. The inspector reviewed compliance in the areas listed above. There were 11 children present and 3 caregivers; the point total was 29. The inspector reviewed 4 children?s records and 3 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Staff must have the Central registry search submitted by the first day of work.

The provider has not submitted the central registry form for one staff who has been employed for more than 2 weeks.
Plan of Correction: I will make sure we fill out the form today.

Annually the provider will review the emergency information with the parent.

Two of the children are missing the annual updates in their files. Child 2 and 3.
