Inspector Notes

An unannounced monitoring inspection was conducted December 7 2023 between 10:45am and 12:00pm. There were 79 children in the care of 17 staff. Children were observed being diapered, washing hands, being fed, playing with magnet blocks, and preparing for lunch. Staff records, child records, medications, the first aid kit, evacuation drills, and the physical plant was observed and inspected. Citations are listed on the violation notice. Based on record review, the center did not ensure that a new staff member obtained documentation of a tuberculosis screening or test no longer than 30 days prior to employment.

Evidence: Staff #3 was hired May 2023 and the documented Tuberculosis screening was dated February 2023.

Based on observation, the center did not ensure that all hazardous substances, such as cleaning materials, are kept in a locked place using a safe locking method.

Evidence: During the walkthrough, a bleach water solution was observed to be on top of the child cubbies.
