During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 04/14/2026, as a part of the licensure period. The on-site inspection began at 11:45 am and ended at 1:45 pm. The inspector reviewed compliance in the areas listed above. There were 59 children present and 13 staff. The inspector reviewed 7 children?s records and 7 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff.

On 4/14/26, staff #1 failed to stay within sight and sound supervision of the preschool class. She was in the bathroom changing one child, while the remaining 9 children in the class were not within her sight for a period of approximately 1 minute. The bathroom was within the classroom, but there was not a line of sight for the teacher to the rest of the class.
Plan of Correction: We will retrain staff on our supervision policy.
Standard 8VAC20-780-500-B
The diapering area shall have a nonabsorbent surface for diapering, and a leakproof storage system that is foot-operated so that neither the staff member's hand nor the soiled diaper touches the exterior surface of the storage system.

The diaper changing table pad in the older infant classroom was ripped so it was no longer nonabsorbent. The Toddler and two year old classrooms had foot-operated trash bins available, however diapers were found in trash bins that were not foot-operated
Plan of Correction: A new diaper changing pad will be ordered, and staff will be retrained on the diaper changing and diaper disposal policies.
