Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 05/11/2021 and concluded on 05/28/2021. The provider was contacted by telephone to initiate the inspection. There were 4 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Violations

There was a broken toilet tissue holder located in the bathroom of the main child care area.
Evidence:
Unused outlets in the bathroom of the main child care area were not covered.
Evidence:
4 of 4 children's records reviewed were missing required information.
1. Child A was missing 1 of 2 required emergency contacts.
2. Child B was missing 1 of 2 required emergency contacts.
3. Child C was missing the employer phone number of the father.
4. Child D was missing the employer phone number of the father.
Evidence:
There were no records of practiced emergency evacuation drills for January 2021, February 2021, March 2021 and April 2021.
