Inspector Notes
An unannounced renewal inspection was conducted today from approximately 10:00am-11:00am. There were 3 children in the direct care and supervision of 2 caregivers. A sample of 3 children?s records, and 2 caregiver's records were reviewed. The children were observed watching television and being dropped off by parents. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. The sworn statement for Caregiver A expired 5/2021.
Evidence:
Child A and Child B did not have documentation of a physical exam on file.
Evidence:
Caregivers A and B did not complete 16 hours of annual training.

A rice cooker and a blender were plugged in and not in use during the inspection.

The record of children A, B and C were missing permission to go of the premises.

Caregiver A stated she was unable to locate the documentation for emergency drills.

The license posted in the facility expired in 2021.
