Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a in-person tour of the program A renewal inspection was initiated on 3/14/22 and concluded on 3/15/22. The provider was contacted by telephone to initiate the inspection. There were 35 children present and 10 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
2Evidence:
1. The most recent health inspection report inspection available for viewing during the inspection was dated 1/5/21.
2. Staff #5 (Program Director) confirmed that the annual health inspection had not been completed.
Evidence:
1. The resilient surface (mulch) around both play structures was less than the required 6 inches in depth
2. There were exposed tree roots at the base of the slide on the smaller play structure.
3. Staff #5 (Program Director) viewed the resilient surfacing (mulch) in the fall zone for the play structure, and confirmed that it was not compliant with the requirements in the standards of six inches of resilient surfacing.
