Inspector Notes

An unannounced monitoring inspection was conducted on 5/22/24 from 10:30am to 12:20pm. At the time of the inspection there were 50 children in care and 8 staff members. Children were observed participating in various activities including center and outdoor play. Records were reviewed for five children and three staff members. Medication, evacuation drills, emergency supplies and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Standard 8VAC20-780-280-B
Based on observation and interviews, it was determined that the facility did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
In the 3-year-old classroom there was a can of Lyson spray disinfectant in an unlocked cabinet.
Plan of Correction: A staff member removed the Lysol during the inspection and placed it in a locked cabinet.
Standard 8VAC20-780-280-H
Based on observation and interviews, it was determined that the facility did not ensure that harmful agents shall not be stored in areas that are accessible to children.
Evidence:
In the 2-year-old classroom there was a container of Mod Podge that read ?extremely flammable, vapor harmful, eye irritant? in an unlocked cabinet that was accessible to children.
Plan of Correction: A staff member removed the Mod Podge during the inspection and placed it in a locked cabinet.
Standard 8VAC20-780-350-B-4
Based on observations, record review and interviews it was determined that the facility did not ensure that the staff-to-child ratio of 1:10 for children ages 3 years old up to school age eligible was maintained.
Evidence:
1.Upon entering the 4-year-old classroom there were 15 children in the care of 1 staff member.
2.The Director confirmed a staff member had stepped out of the classroom leaving 1 staff member to supervise 15 children.
Plan of Correction: Director coached the staff on procedures to stay in ratio if need to step outside the classroom.
Standard 8VAC20-780-60-A
Based on review of children?s records and interviews, it was determined that the center did not ensure that each child?s record contains all required information.
1.The records for child #1 and child #2 did not contain the address of the emergency contacts.
2.The Director confirmed this information was not in the record.
Plan of Correction: Director emailed the parents requesting the corrections be made to childrens files.
