During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 7/18/25 as a part an Intensive Plan of Correction (IPOC). The on-site inspection began at 10:05am and ended at 11:35am. The inspector reviewed compliance in the areas listed above. There were 41 children present and 10 staff. The inspector reviewed 3 staff records on site. This inspection included document review, tour of the facility, interviews,and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat Violation
Hazardous substances must be in a locked place that prevents access by children. In a preschool classroom there were cans of shaving cream and spray paint in an unlocked cabinet, out of reach of children.
Plan of Correction: The facility responded: New locks will be installed in the classroom with a universal key for all. Locks were installed 7/24/25.

Repeat Violation
In areas used by preschool age or younger children, electrical outlets shall have protective covers. There was one uncovered electrical outlet in a preschool classroom, within reach of children.
Plan of Correction: The facility responded: A plan is already in process to remind housekeeping to plug outlets after use.
