During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site monitoring inspection was initiated and completed on 10/23/25. The on-site inspection began at 9:45am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 63 children present and 8 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/14/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a safe and operable condition. In the bathroom of a four-year-old classroom there was peeling paint on the wall near the toilet, within reach of children.
Plan of Correction: The facility responded: Put in a work order.
Standard 8VAC20-780-290-A-3
Repeat Violation
In areas used by preschool age or younger children, electrical outlets will have protective covers. There was an uncovered outlet in a three-year-old classroom, within reach of children.
Plan of Correction: The facility responded: Retrained staff and passed out more covers.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards (6 inches) and be free of hazards. The mulch in front of the slide measured approximately 2 inches in depth and a root was protruding in the fall zone.
Plan of Correction: The facility responded: Reported to supervisor.
Standard 8VAC20-780-340-D
Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications. Staff #2, who did not meet lead teacher qualifications, was alone for approximately 25 minutes with a group of four-year-old children in the classroom and on the playground.
Plan of Correction: The facility responded: Staff is no longer with us. Retrained staff to be aware of this.
Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff.
1.Staff #1, who was responsible for supervising eight (four-year-old) children left the children on the pool deck without any other staff from the licensed childcare program.
2.Staff #2, the only staff member on the playground, turned her back to the play area to attend to another child. Staff #2 was unaware that a child, out of her line of vision because her back was turned, was hanging upside down from a metal bar that extended across the slide.
3. Staff #3 and Staff #4 were unaware that a child left the classroom to use the bathroom in the adjoining classroom.
Plan of Correction: The facility responded: 1. Swim lessons have been cancelled. 2. Staff have been retrained. 3. A new plan has been put in place and staff have been retrained.
Standard 8VAC20-780-460-A
When children are participating in a swimming activity there will be at least two staff members supervising. Staff #1 was the only program staff member supervising eight (four-year-old) children during a swim lesson.
Plan of Correction: The facility responded: Cancelled swim lessons.
Standard 8VAC20-780-480-B
The center shall maintain written permission from the parent of each child who participates in swimming. On 10/23/25 Child #1 participated in a swimming lesson. The swimming participation form completed by the parent indicated that the child was a non-swimmer and the parent did not give permission for the child to participate in swimming activities.
Plan of Correction: The facility responded: The parent was contacted during the inspection. The parent updated the permission form giving permission for the child to participate in swimming/wading activities.
Standard 8VAC20-780-530-A
At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid. Staff #2, alone with 8, four-year-old children was not certified in CPR and first aid.
Plan of Correction: The facility responded: Staff is no longer with us. All other staff have this.
Standard 8VAC20-780-560-G
When food is brought from home the food container must be clearly dated. There were 5 lunchboxes and 4 water bottles in a preschool classroom that were not dated.
