Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 4/20/21 and concluded on 4/21/21. The provider was contacted by telephone to initiate the inspection. There were three children present with the provider. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and a violation was documented on the violation notice issued to the facility.

Based on review of documentation and interview with the provider, the provider did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for Child B, with diagnosed allergies to peanuts, eggs, hazelnuts, and fish, did not contain an allergy care plan.
Plan of Correction: The parent submitted the allergy care plan from the doctor today.
