Yes

Areas Reviewed

8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Inspector Notes

An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted on 4/6/22 from 9:30am to 11:00am in conjunction with a monitoring inspection (see that inspection report for details). The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Two caregiver records were reviewed for documentation of the mandatory Virginia Preservice Training and current certification in First Aid and CPR. There are no children currently enrolled with diagnosed food allergies or medication. The last lockdown drill was documented on 8/16/21. If you have questions regarding this inspection, please contact the Licensing Inspector, Laura Brindle, at laura.brindle@doe.virginia.gov or 540-905-2062.

Violations

3
Standard 8VAC20-790-180-B
Based on review of documentation and interview with the provider, the provider did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: At approximately 9:35am on 4/6/22 the attendance record contained documentation of six children in care. There were only four children in care. The provider reported that two children had left for public school. Interview revealed that the attendance record documents the children who attended for the day, rather than documenting the current children in attendance by documenting arrival and departure.
Plan of Correction: I now understand that the attendance should document arrival and departure to show who is currently in care. Moving forward I will add the times of arrival and departure for each child.
Standard 8VAC20-790-250-D-1
Based on record review and interview, the provider did not ensure that all caregivers had current certification in cardiopulmonary resuscitation (CPR). Evidence: The record for Caregiver #1 did not contain documentation of a current CPR certification and the provider reported that the certification had expired.
Plan of Correction: Caregiver #1 is not my regular assistant. I will make sure Caregiver #1 gets re-certified if they will continue to act as an assistant.
Standard 8VAC20-790-250-D-2
Based on record review and interview, the provider did not ensure that all caregivers had current certification in first aid. Evidence: The record for Caregiver #1 did not contain documentation of a current first aid certification and the provider reported that the certification had expired.
Plan of Correction: Caregiver #1 is not my regular assistant. I will make sure Caregiver #1 gets re-certified if they will continue to act as an assistant.
