Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
An unannounced code compliance inspection was initiated on 1/27/2022 and concluded on 1/28/2022. The person in-charge was contacted by telephone and a virtual inspection was conducted. There were 7 children present, ranging in ages from two to five, with two staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision and procedures. A total of two children?s records and one staff?s record was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the center failed to obtain fingerprint-based criminal history check determination letters prior to the first day of employment.

Evidence:

Staff 1's hire date was 6/28/21. The fingerprint-based criminal history check was dated 8/11/21.
Plan of Correction: In the future staff will not be hired until the fingerprint-based criminal history check is received.

Based on record review and interview, the center failed to have have a completed central registry record check by the end of the 30th day of employment.

Evidence:

Staff 1's hire date was 6/28/21. The findings of the central registry background check is dated 9/22/21.
Plan of Correction: In the future central registry background checks will be mailed upon hire.
