Inspector Notes
An unannounced monitoring inspection was conducted today from 10:20am-11:10am. There were 12 children directly supervised by 2 caregivers. The child care area, 3 caregiver records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today and there is no caregiver at the family day home who is currently authorized to administer medications.
The provider was not present during the inspection and the caregivers present were supervising children in excess of the supervision guidelines. The caregivers on site did not have access to children or staff files and the provider of another center was called to provide the inspector access to the records.
Children were observed playing freely, napping in cribs and eating snack. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Violations
10Evidence:
The turf covering on one of the cement steps leading to the family day home entrance was observed to be ripped at the riser and hanging loosely above the step beneath it, creating a tripping hazard.
Evidence:
At the beginning of the inspection, approximately 10:20am, the attendance sheet for the day had 12 children's names listed and there were 12 children present at the family day home. At 10:36am the attendance document was reviewed and Child 6 and Child 7's names had been blacked out. Child 6 and Child 7 remained in attendance throughout the duration of the inspection.
Evidence:
The supplemental page of licensing inspections dated 7/24/23, 2/8/23, and 5/22/22 were observed posted on the bulletin board. The page contains personally identifying information about children enrolled in the family day home and is labeled "CONFIDENTIAL DO NOT POST".
Evidence:
Four cribs were observed with loose bottom sheets.
Evidence:
On the date of inspection there were 12 children totaling 39 points under the supervision of 2 caregivers. The caregivers exceeded the point total by 7 points.
Evidence:
There was not a record for Child 6 or Child 7 on the date of inspection.
Evidence:
1. Child 2 and 3's record did not contain an annual review of information.
2. Child 3 ands 5's record did not contain a health insurance policy number.
3. Child 5's record did not list the name, address, and phone number for either listed parent's place of employment.
Evidence:
Child 4 was observed in a bouncy seat at approximately 10:20am and remained there until at least the end of the inspection at 11:10am.
Evidence:
There was one documented shelter-in-place drill conducted in 2023.
Evidence:
Children were observed walking while drinking out of sippy cups.
