Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/1/2024 and completed on 10/1/2024, as a part of the provisional licensure period. The on-site inspection began at 10:00am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were 11 children present and 3 caregivers; the point total was 3. The inspector reviewed 5 children?s records and 4 caregiver/household member(s) records on-site.

This inspection included : document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/08/2024. A POC submitted after this date will not appear on the public website.

Information contained in a child's record shall be privileged and confidential. The provider shall not distribute or release information in a child's record to any unauthorized person without the written consent of the child's parent. The supplemental page from the previous licensing inspection was posted in public view in the family day home. THe page contains identifying information about children in care.
Plan of Correction: Document containing information was removed.

Children's records shall be maintained with all required information. Child 4's record did not contain the child's health insurance policy number.

Medication shall be kept in a locked place using a safe locking method that prevents access by children. Children's medication was observed stored in an unlocked cabinet in the child care area.
Plan of Correction: Medication will be placed in a locked box/cabinet.

Bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care. Four bottles were observed without a label with the date.
