During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on August 6, 2025 and completed on August 6, 2025. The on-site inspection began at 9:50am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were 11 children present and 3 caregivers; the point total was 34. The inspector reviewed 4 children?s records and 4 caregiver/household member records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 14, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-140-B-3
Providers are required to have a current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.

Caregiver 1's certification has been expired for seven months.
Plan of Correction: I have misplaced my certificate. As soon as I find it I will send it.

If I cannot find it I will get a new certificate with the class.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department
Caregiver 2's Tuberculosis Screening is overdue by seventy-two days.
Plan of Correction: Appointment has be scheduled to get updated TB form.
Standard 8VAC20-800-60-B
A child's record shall contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds.

Child 3's record did not contain written authorization to obtain emergency medical care.
