Inspector Notes
An unannounced monitoring inspection was initiated on October 30, 2025 and completed on November 10, 2025 in response to a self-report received by the licensing office on October 30, 2025 relating to forbidden actions. The inspector reviewed one child?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
