Inspector Notes

An on-site renewal inspection was completed on May 11, 2022. There were 9 children present, ranging in age from 11 months through 5 years, and in the care of the provider and two assistants (23 Points). The inspector reviewed compliance in the areas of administration, caregiver training, physical environment, care of children, preventing the spread of disease, emergencies, medications and nutrition. A total of 2 children's records and 3 caregivers records were reviewed.

The information gathered during the inspection determined violations with applicable standards or law and were noted on the violation notice.

Based on record review and discussion with provider, the provider failed to obtain an out-of-state sex offender registry check for one new staff member from the state (outside of Virginia) in which she currently resides.

Evidence:
1. The record for Assistant #2 did not contain an out-of-state search of the sex offender registry from the state, other than Virginia, where she is currently residing. Date of employment for Assistant #2 was 02/28/2022.
2. The provider reported that she had not obtained the registry search for the assistant and was unaware of this requirement.
Plan of Correction: The provider will obtain the search as soon as possible and from now on she will complete all required out-of-state background checks when hiring someone who has lived outside of Virginia within the past five years.
Standard 8VAC20-800-180-A
Based on review of the provider's record and confirmation from the provider, the provider did not obtain a follow-up tuberculosis (TB) screening within two years of the most recent one.

Evidence:
1. The record of the provider contained an updated negative TB screening dated 05/10/2022. The date of her most recent previous TB screening was 01/14/2020.
2. The provider confirmed she was late getting her TB screening.
Plan of Correction: The provider obtained the updated negative TB screening as soon as she realized she had missed the date.
