Inspector Notes

An unannounced monitoring inspection was conducted on 11/4/2022 from 9:30 a.m. to 10:30 a.m. There were 4 children present, ranging in ages from 15 months to 4 years old, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 9 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of records and interview, the provider failed to obtain a physical examination prior to a child?s attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #5?s first day of attendance was 5/23/2022. There was no physical examination on file for Child #5.
2. Staff #1 confirmed that Child #5 did not have a physical examination on file.
