During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was conducted on 5/1/2025. The on-site inspection began at 11:22 am and ended at 4:00 pm. The inspector reviewed compliance in the areas listed above. There were 199 children present and 36 staff. The inspector reviewed 10 children?s records, 10 staff records, and 4 officer records on-site.This inspection included document review, a tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 5/9/2025.
A POC submitted after this date will not appear on the public website.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #8, employed for over 10 months, did not have a central registry check from one state.
Plan of Correction: Request was made, results were not returned. New request was made on 5/2/2025. Staff is not alone until request is received.
Standard 8VAC20-780-550-P
Minor injury records shall include the following: 1. Date and time of injury; 2. Name of injured child; 3. Type and circumstance of the injury; 4. Staff present and treatment; 5. Date and time when parents were notified; 6. Any future action to prevent recurrence of the injury; 7. Staff and parent signatures or two staff signatures; and 8. Documentation on how parent was notified.

Child #11, child #12, and child #13 had minor injury reports that were missing the future action to prevent recurrence of the injury.
Plan of Correction: Correct procedures for injury reports were reviewed with admin and staff in the school age building.
