When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.
On 11/06/2025, Staff #13 was observed supervising a combined group of 17 preschool age and school age children. Administration acknowledged that Staff #13 was seven children over the required preschool ratio of 1:10 for approximately 15 minutes before Staff #1 arrived to supervise.

Plan of Correction: Parents were aware but we didn't have signatures. Since then signatures on a waiver were obtained and placed in the children's files.
