Inspector Notes

An unannounced monitoring inspection was conducted 2/22/2022 from 10:50am to 12pm. There were approximately 30 children directly supervised by 5 staff. The physical plant, 4 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play and individual activities, interacting with caregivers, playing in centers, preparing for and eating lunch. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on review, the facility did not ensure that they obtained documentation of a physical examination for each child in prior before the child's first day of attendance, or within 30 days of the first day of care.

Based on observation, the facility did not ensure that electrical outlets accessible to children have protective covers.
