Inspector Notes

An unannounced monitoring inspection was conducted on 12/13/2023 from 10:05am to 11:05am. There were 42 children ages 15 months to age 5 years old supervised by 8 staff. The physical plant, programming, attendance, menus, 4 staff records, 2 children?s records, and emergency drills were reviewed. Children were observed participating in literacy activities, writing and transition activities. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-780-280-B
(Repeat Violation) Based on observation, the center did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
During inspection in the Prekindergarten classroom, there was a cabinet with an open door that contained 1 container of sanitizer, 1 bottle of air freshener, 3 containers of disinfecting wipes and a container of whiteboard wipes.
Plan of Correction: All cabinets with hazardous substances will remain locked.

Based on review of 2 child records, the center did not ensure annually that parent confirmation of the required information in the child record is up to date.
Evidence: The file for Child #2 did not have documentation that the parent had confirmed the updated record annually.
Plan of Correction: A new contract form was created. Each parent updated their contacts, signed and dated the documents.

Based on review of 2 child records, the center did not obtain parent authorization for emergency medical care should an emergency occur when the parent cannot be located immediately.

Evidence: The record for Child #2 did not contain parent authorization for emergency medical care in case of an emergency when the parent cannot be reached.
