Inspector Notes
An unannounced monitoring inspection was conducted on 12/13/2023 from 10:05am to 11:05am. There were 42 children ages 15 months to age 5 years old supervised by 8 staff. The physical plant, programming, attendance, menus, 4 staff records, 2 children?s records, and emergency drills were reviewed. Children were observed participating in literacy activities, writing and transition activities. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

3Evidence:
During inspection in the Prekindergarten classroom, there was a cabinet with an open door that contained 1 container of sanitizer, 1 bottle of air freshener, 3 containers of disinfecting wipes and a container of whiteboard wipes.
Evidence: The file for Child #2 did not have documentation that the parent had confirmed the updated record annually.
Evidence: The record for Child #2 did not contain parent authorization for emergency medical care in case of an emergency when the parent cannot be reached.
