Inspector Notes

An unannounced monitoring inspection was conducted 5/30/2024 from approximately 2:40pm-4:05pm. There were 9 children (32 points) directly supervised by 3 caregivers. The home, 4 caregiver records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed doing table top puzzles, diaper being changed and waking from naps. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-800-330-A
Based on observation, a landline telephone, excluding a cordless or cell phone, was not available, operable, and accessible during the family day home's hours of operation.
Evidence: A landline phone was not operable during operation hours of the day home.
Plan of Correction: Provider found a telephone that does not require electricity in the home. To contact telephone company regarding phone jack repair/function.
Standard 8VAC20-800-780-B
Based on record review, the provider did not review emergency contact information required in
8VAC20-800-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the annual review.
Evidence: Child #3 and Child #5 records did not have parent' signed acknowledgment of the annual review of emergency contact
Plan of Correction: Provider will have children's parents review, update as needed and sign emergency information.
Provider will ensure emergency contact information is reviewed and acknowledgement is signed by parents annually.
Standard 8VAC20-800-90-C
Based on record review, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements.
Evidence: Child #4's record did not contain immunizations, once every six months prior to second birthday. (First Day 8/17/2021, DOB: 9/2020, date of last immunization 1/28/22)
Plan of Correction: Provider will request accurate immunization record and review upon receipt for all children.
