Inspector Notes

A monitoring inspection was conducted on 5/26/21. Due to the COVID 19 pandemic the inspector was not on site and the inspection was conducted via video application as well as email and phone correspondence. During the virtual observation there were 35 children supervised by 8 staff. The facility, 3 staff records, 3 children?s records, evacuation drills, emergency supplies, medication and policies were inspected. Children were observed playing freely and participating in circle time and eating lunch of Quinoa and vegetable soup with fruit and milk. There was an adequate supply of books, toys, and materials for the children. Areas of non- compliance are indicated in the violation notice

Based on observation it was determined that all areas of the facility were not maintained in a clean, safe and operable condition.

Evidence: During the inspection the radiator in a bathroom used by children had peeling paint and a rusting grate exposed.
Plan of Correction: I will contact maintenance to correct it and monitor child with bathroom is in use.
