Inspector Notes

A subsidy inspection was initiated on May 25, 2022 it was conducted inperson and hybrid . There were were 18 children in care. The inspector reviewed four children's records and five records submitted by the vendor to ensure compliance with the health and safety regulations. The Licensing Inspector observed children preparing nap, napping, eating lunch and participating in movement activity. The information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were issued.

Based on record review, the vendor did not ensure that staff had been evaluated by a health
professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) at the time of employment and prior to coming in contact with children.
Evidence: There was no documentation of a TB test or screening for Staff #5 (date of employment
01/05/2022).
Plan of Correction: Staff #5 recieved results of TB test 6/23/2022. Vendor sent the document to inspector.
Standard 8VAC20-790-760-B-1
Based on observation and staff interview, the vendor did not ensure that a nonabsorbent surface for diapering or changing should be used and the diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use.

Evidence: As confirmed by Staff #1, the vendor was using a cloth diapering surface. Confirmed with staff #1. Two children were observed being changed by Staff #2 and Staff #6 on the cloth diapering surface. Between changes, the diapering surface was not cleaned or sanitized.
Plan of Correction: Vendor sent picture of a new diapering surface.
