Inspector Notes

An unannounced on-site monitoring inspection was conducted on 12/27/2022. There were 59 children, ages 8 months-12 years, and 10 staff members present during the inspection. The inspector observed the following: nap, and eating snack. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: Portal for CPS backgrounds and orientation form found on the DOE website.

Based on review of five staff?s members, the center failed to ensure that all out of state background checks were obtained.

Evidence
Staff # 3 lived outside of the state of Virginia in the past five years in a non NFF state (national fingerprint file). The Staff is required to have a criminal background prior to starting the position. The staff?s start date was 10/27/2022.

Standard 8VAC20-780-350-C
Based on observation, the center failed to ensure that the ratio was meet when children are in ongoing mixed age group, the staff-to children ratio and group size is applicable to the youngest child in the group.

1.At arrival to the center the school age classroom had 27 children present with one staff person. The youngest child in the grouping was 3 years old. Approximately 10 minutes after arrival two children were picked up making the total number of children 25 with one staff person. The total number of staff for this grouping of children is 10 children to one staff.

Based on review of five children?s records the center failed to ensure that all children enrolled had the three parental agreements signed by the parent.

Evidence:
1.Child # 1 did not have documentation of the signed three parental agreements in the child?s record. The child was enrolled on 10/10/2022.
