Inspector Notes
An unannounced on-site renewal inspection was conducted on 08/28/2023. There were 46 children, ages 3 months-5 years, and 7 staff members present during the inspection. The inspector observed the following: free choice activities, lunch and nap time. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: sleeping requirements and application. The inspector arrived at 10:45 AM and departed at 1:15 PM.

Violations
5Evidence:
1. The record for Child # 5 contained documentation that the first date of attendance
was 03/06/23. There was no documentation of a physical in the record on the day of the inspection.
Evidence:
At 10:50 AM, a 7-month-old was sleeping on the floor in the play area laying in a boppy pillow. The LI returned to the infant room at 12:32 and the infant was still sleeping laying on the boppy pillow.
Evidence:
1.Observed staff # 3 caring a soiled diaper from the bathroom into the classroom to discard. The staff person proceeds to the lunchroom and placed a child's lunch on a plate without washing hands after discarding the soiled diaper.
Evidence:
Observed four infants sitting in highchairs eating lunch without safety belts being fastened. The safety belts were observed hanging down the side of the highchair.
Evidence:
1. The record for Child # 1 did not include the following information: allergies and action to be taken.
2. The record for Child #5 did not include the following information: address and phone number of one of the two designated people to call in an emergency if a parent cannot be reached.
