Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated on August 31, 2021 and concluded on August 31, 2021. There were eight children present with two staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of two caregivers' records and interview, the vendor did not ensure each caregiver completed the department's health and safety update course annually.

Evidence:

1. The record of caregiver #1 contained documentation of the caregivers' completion of the required Virginia Annual Health and Safety Update Training dated 06/28/2020. Caregiver #1 did not have documentation of completing the updated health & safety training within 12 months of the original training. Caregiver #1 did not complete the training by June 27, 2021 as required.
2. The record of caregiver #2 contained documentation of the caregivers' completion of the required Virginia Annual Health and Safety Update Training dated 06/14/2020. Caregiver #2 did not have documentation of completing the updated health & safety training within 12 months of the original training. Caregiver #2 did not complete the training by June 13, 2021 as required.
Plan of Correction: Per the provider: We will complete the updated health and safety training every year as asked.
