Inspector Notes
This inspection was conducted by licensing staff using an alternate hybrid protocol.
A renewal inspection was initiated on 02/04/2022 and concluded on 02/08/2022. The provider submitted requested documentation to the inspector on 02/04/2022 for review and the inspector conducted an unannounced inspection on-site on 02/08/2022 from approximately 8:30am to 10:00am. On 02/08/2022, there were four children present with two caregivers supervising for a value of ten points. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records, two caregiver records and two household member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
3Evidence:
The CPR certification for caregiver #1 expired on 11/02/2021. Caregiver #1 stated she did not have current CPR as she had not renewed her CPR certification as of yet.
Evidence:
Documentation of the most recent tuberculosis screening for household member #1 was dated 05/16/2019 exceeding the requirement to renew every two years.
Evidence:
The training log for caregiver #1 had a total of 15 hour of annual training for the period 05/31/2020 through 05/30/2021 when 16 hours was required.
