Inspector Notes
A monitoring inspection was conducted on July 17, 2023 from approximately 8:37 am to 10:05 am. There were three children in care, ranging in ages from 2 to 3 years with one caregiver supervising for a total of 6 points.
The inspector reviewed compliance in the areas of administration, care of children, physical plant, physical equipment and environment, staffing and supervision, programming, special care and emergencies and nutrition.
A total of five child records, and two caregiver records were reviewed. The children were observed playing with toys during the inspection. The caregiver's interactions with the children were positive. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Violations
4Evidence:
The bathroom used by children in care had brown dirt stains around the bottom of the toilet and sink.
Evidence:
The record for Child #3 failed to include a second emergency contact person to call if a
parent cannot be reached and the address for the one emergency contact person documented.
Evidence:
1. During the inspection, the battery-operated flashlight was observed as not working.
2. The substitute caregiver acknowledged during the inspection, that the flashlight was not operating and was not able to locate a working flashlight during the inspection.
