During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated on September 10, 205, and completed on September 10, 2025. The Department entered into a Consent Agreement with the facility/home on February 4, 2025. The on-site inspection began at 2:00 pm and ended at 3:30 pm. The inspector reviewed compliance in the areas listed above. There were four children present and one caregiver; the point total was 8. The inspector reviewed six children?s records and two caregiver/household member(s) records on-site.
This inspection included document review, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises. A copy of the Consent Agreement dated February 4, 2025, including the Notice of Intent to Deny the License dated October 7, 2024, and violation notices listed in the Notice of Intent dated May 15, 2024, July 24, 2024, and September 10, 2024, and a copy of the Final Compliance Plan submitted to the Department on November 22, 2024 were not posted in the family day home.
Plan of Correction: Per the provider: I will hang the postings on the door now and make sure they remain there.
