Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 9:00 am through 10:00. am There were a total of six children (12 points) observed with one caregiver. A total of four children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, games, hand washing, and other organized activities. Based on record review and interview, the provider did not obtain subsequent tuberculosis screening for each caregiver.
Evidence:
1. Caregiver #1 did not obtain a current report of tuberculosis screening every two years from the date of the first screening. The tuberculosis screening in the record was dated 06/04/2020 and expired on 06/04/2022.
2. Caregiver #1 confirmed that a current tuberculosis screening was not obtained.
Plan of Correction: The caregiver will have a new tuberculosis screening completed.
