Inspector Notes
An unannounced monitoring inspection was conducted on January 3, 2024 between the hours of 9:00am through 10:15am. There were a total of 8 children (16 points) observed with one caregiver. A total of four children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, coloring, drawing, and other organized activities. Violations
3Evidence:
1. Emergency evacuation drills were not practiced the months of November and December.
2. Caregiver #1 confirmed that the emergency evacuation procedures were not practiced for the months of November and December 2023.
Evidence:
1. Child # 4's record (Start date-09/01/2023) did not have documentation that the child had been adequately immunized. The results from the physical examination from a physician was completed, but the immunization record was blank and did not contain immunization dates.
Evidence:
1. The menu for the current one week period was not dated.
