Inspector Notes
A Monitoring inspection was conducted this date. A total of 11 children (26pts) were present with 3 staff. The children were seated at the table with staff engaged in playing with playdoh. The children were busy making pretend food with the playdoh. One infant was sleeping in the crib room and 2 older children were playing a board game on the floor. Walk thru of the program area was made. A nice transition from the table activities to the floor was made where the children began playing with toys and equipment available to them. Morning snack was served which consisted of crackers and yogurt. Staff were observed assisting with spoon feeding the younger children in high chairs. A total of 5 children's records, all staff records and 1 household record was reviewed. Previous violations were reviewed and the corrective actions were verified. Children began eating morning snack without staff ensuring their hands were washed before eating.
Evidence:
1. Child #1's immunizations did not include documentation from a physician that the child has been adequately immunized.
2. Child #3 began care on 5/3/2022. Immunizations were not obtained until 5/11/2022.
3.Child #4's immunizations dated 3/3/2023 did not include documentation from a physician that the child has been adequately immunized.
Evidence:
Child #3 is under the age of 2 years. Additional immunizations were not obtained every six months. The last documented immunizations on file is dated 5/11/2022.
