Inspector Notes

An unannounced Monitoring inspection was conducted today. A total of 7 children of 3 staff (20pts) The children had just finished lunch and transitioned to the floor to free play. Observation of the programming space, bathroom, crib room and food preparation area. The children were observed playing "kitchen" on the floor with 2 staff and the Provider was observed holding and bottle feeding one child. Handwashing and bathroom procedures were observed before the children transitioned to nap time. Two new cribs purchased since the last inspection. A total of 5 children's records, 3 staff and 1 household were reviewed. The previous violations from the last inspection were reviewed and one repeated violation found along with new violations found out of compliance today. Based on child record review, 2 out of 5 records were incomplete.
Evidence:
Chiild #1 and 2's file was missing the start date of care.
Child #4'f file was missing two complete emergency contacts.
Plan of Correction: This information will be obtained from the parents.
Standard 8VAC20-800-90-A
Based on child file review, the provider failed to obtain documentation that the child has been adequately immunized before 2 children began attendance.
Evidence:
Child #1 began care on 10/01/23, immunizations on file were dated 10/03/23.
Child #4 began care on 10/02/23, immunizations were dated 10/04/23.
Plan of Correction: No child will began care without documentation of immunizations.

REPEAT
Based on child file review, documentation of additional immunizations were not obtained every six months for 1 child under the age of 2 years.
Evidence:
Child 5 is under the age of 2 years. The last documented immunizations were date 6/22/23.
Plan of Correction: This child is due for a check up. I will obtain a copy of the immunizations from the parent.
