During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was completed on 03/20/2025, as a part of the licensure period. The on-site inspection began at 10:00 am and ended at 11:15 am. The inspector reviewed compliance in the areas listed above. There were 11 children present and 3 caregivers; the point total was 29. The inspector reviewed 4 children?s records and 4 caregiver/household members records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1Child #1 and Child #3 were missing the policy number for their medical insurance. Child #1 was also missing documentation of the annual review of emergency contact information, and a signed acknowledgement of the receipt of the provider information.
