Inspector Notes
An unannounced monitoring inspection was initiated and completed on May 6, 2025 in response to a self-report received by the licensing office on 4/30/2025 relating to forbidden action. The inspector reviewed 1 children?s record and 1 staff record on-site. This inspection included document review, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on June 4, 2025. A POC submitted after this date will not appear on the public website.
Violations
1On April 24, 2025, Staff #1 pulled Child #1 (age 3) by the arm to stand the child up. Staff #1 proceeded to pull child #1 by the arm across the room and sat child #1 down by pulling the child?s arm. Staff #1 then stated, ?I?m tired of you little girl?.
