Inspector Notes

An unannounced renewal inspection was conducted on 5/13/2024 from 8:40am-10:50am. There were 7 children directly supervised by the provider. The home, 1 caregiver record, 3 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. Children were observed having breakfast and playing freely. Diapering and handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the provider did not obtain a current report of tuberculosis screening every two years for 1 household member.

Evidence:
1. The most recent tuberculosis screening in the record for Household Member #1 was dated 4/28/2022.

Repeat Violation

Based on observation and provider interview, the provider did not provide all required daily age-appropriate activities.

Evidence:
The provider is not providing the children with opportunities for vigorous outdoor play daily. The provider stated she has not been taking the children outside during the day, as the area she typically uses, the backyard of the home, is not available due to the space being utilized for storage.
Plan of Correction: The backyard will be cleaned so that the children can play outside.

Based on record review, 1 of 3 children's records reviewed did not contain all required information.

Evidence:
1. The record for Child #1 did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
Plan of Correction: The authorization was signed by the parent.

Based on record review, the provider did not obtain the parent's written acknowledgement of the receipt of information for 1 child.
