Inspector Notes
An unannounced renewal inspection was conducted on 5/13/2024 from 8:40am-10:50am. There were 7 children directly supervised by the provider. The home, 1 caregiver record, 3 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. Children were observed having breakfast and playing freely. Diapering and handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Violations
7Evidence:
1. The most recent tuberculosis screening in the record for Household Member #1 was dated 4/28/2022.
Based on observation and provider interview, the provider did not provide all required daily age-appropriate activities.
Evidence:
The provider is not providing the children with opportunities for vigorous outdoor play daily. The provider stated she has not been taking the children outside during the day, as the area she typically uses, the backyard of the home, is not available due to the space being utilized for storage.
Evidence:
1. The record for Child #1 did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
Evidence:
1. The record for Child #2 did not contain written acknowledgement of the receipt of the provisions of the family day home's emergency preparedness and response plan
Evidence:
1. The most recent review of the emergency plan was dated 5/3/2023.
Based on record review, emergency evacuation procedures were not practiced monthly.
Evidence:
1. An evacuation drill was not completed in April 2024.
Evidence:
1. Meals and snacks are provided by the family day home.
2. The posted menu was dated for the previous week (5/5-5/10)
