Inspector Notes

An unannounced on-site renewal inspection was initiated and completed on September 9, 2024. The on-site inspection began at 9:45am and ended at 10:15am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 14. The inspector reviewed 5 children's records and 2 caregiver records on-site. This inspection included document review, tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the family day home failed to obtain documentation of additional immunization for a child every six months for children under the age of two.
Evidence: The record for Child #4 (DOB 8/12/222) last immunization update was dated 2/14/24.
Plan of Correction: I will contact the parent s today to get them to bring the updated immunization record.
