During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site monitoring inspection was initiated and completed on March 12, 2025. The on-site inspection began at 10:55amand ended at 11:57am. The inspector reviewed compliance in the areas listed above. There were 5 children present and two caregivers; the point total was 10. The inspector reviewed 3 children?s records and three caregivers/household members records on-site. This inspection included document review, tour of the home, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-210-B
The caregivers shall obtain a minimum of sixteen clock hours of training annually.
Caregivers #1 and #2 only completed 10 clock hours of training in 2024.
Plan of Correction: We will complete a minimum of sixteen clock hours of training this year.
Standard 8VAC20-800-430-A
Children under two years of age and children over two years of age who are not developmentally ready to climb or descent stairs without supervision shall not have access to stairs. The outside deck stairs are accessible to children under two years of age. Child #3, enrolled for two months, is under the age of two years.
Plan of Correction: I will have a gate installed on the stairs to prevent access to the children by March 24, 2025.
Standard 8VAC20-800-60-B
A child's record shall contain name, address, and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. The file for Child #3 who had been enrolled for six weeks, did not have addresses for the emergency contacts.
Plan of Correction: I will have the parents provide the address information today when they pick up their child.
